Safeguard Your Finances: Know Your Provider Enrollment Revalidation 

Attention: Provider Enrollment and Credentialing Teams

Ensure you revalidate before your deadline to prevent a "Stay of Enrollment" or claim rejections, which could result in the deactivation of your Medicare billing privileges.

Here's how to find your revalidation due date:

  1. Medicare Revalidation List Tool
  2. Revalidation notice mailed by National Government Services
  3. Internet-Based Provider Enrollment, Chain and Ownership System (PECOS)
    • Log in with your user ID and password.

IMPORTANT: If your revalidation date is not assigned or is TBD, unsolicited revalidation applications submitted more than seven months in advance will be returned.

You can revalidate your Medicare enrollment either electronically via PECOS or by submitting the appropriate CMS-855 paper application. Current forms are available on our website under Resources > Forms.

Already Sent Your Revalidation Application?

Check the status of your application using our Check Provider Enrollment Application Status Tool on our website. Go to Resources > Tools & Calculators.

Remember to...

  • Pay the application fee. Use the Medicare Provider Enrollment Application Fee Decision Tree Tool to check if a fee is necessary.
  • Ensure an authorized or delegated official signs and dates the CMS form(s). Each newly added official will need to be added under section 6 on the paper application or under individual control topic in PECOS. Unauthorized personnel cannot sign.   • Include all required documents, such as licenses, certifications, degrees, void checks, sales agreements, and more, when applicable.
  • Verify that the LBN matches the IRS documentation, complete with any suffixes, on:
    • CMS application
    • Bank account
    • NPPES (NPI Numbers)
    • CMS-588 EFT form
    • State registration websites
