2024 Calendar Year 1099 Form Information
National Government Services will mail all 1099 Forms for calendar year 2024 no later than 1/31/2025.
- Medicare providers and beneficiaries, who are serviced by NGS regardless of their location, will receive a single Part A and Part B combined Form 1099 from NGS for calendar year 2024.
- 1099s will be available in our system after 1/31/2025; we ask providers to wait until the first week of February, before submitting a request for a copy of their 1099.
- If you are a NGSConnex LSO you will be able to obtain a duplicate copy of your 1099 in NGSConnex.

- What should I do if I did not receive my 2024 1099?
Answer: All payments you received from our Medicare Parts A and B contracts, for each individual Tax ID number, are under a single 1099. Please check with all your office sites. If your mailing address is incorrect or not current on our files, then a new CMS Form 855 is needed. The CMS-855 can be found on the forms page of our website under Resources. Send the completed 855 to the respective mailing address below, addressed to NGS Provider Enrollment.
Note: If you received less than $600 from NGS for calendar year 2024, no 1099 will be issued or sent to you.
- What if I disagree with the amount on my 1099?
Answer: Your 1099 includes all payments you received from our Medicare Parts A and B contracts for each individual Tax ID number. Further, the amount on the Form 1099 may not match the amount of the payments received. If you still disagree with the amount, please contact the NGS Provider Contact Center for your jurisdiction and line of business, starting in February 2025.
- What if I need a duplicate 1099 for the current year or a prior year?
Answer: For current year requests, the NGSConnex LSO can obtain a copy via the NGSConnex portal. For prior year requests, please fax a perfected FOIA request to fax number 315-442-4269.
- What if my legal name or TIN has changed?
Please note that a TIN can be an individual’s SSN or a business’ EIN.
Answer: A new CMS Form 855 is needed. The CMS-855 can be found on the forms page of our website. Send the completed 855 to the respective mailing address below, addressed to NGS Provider Enrollment.

Per IRS regulations, for mail distribution returns, there has to be a “Returned” on the envelope with the date received.
