UTN Alert: Implanted Spinal Neurostimulator Trial and Permanent UTN
The National Government Services Medical Review Department initiated prior authorization reviews for the following outpatient department procedures: Panniculectomy, Rhinoplasty, Vein Ablation, Blepharoplasty, Botulinum Toxin, Cervical Fusion and Implanted Spinal Neurostimulators.
Providers who plan to perform both the trial and permanent implantation procedures using CPT 63650 in the hospital OPD will only be required to submit a PAR for the trial procedure. To avoid a claim denial, providers must place the UTN received for the trial procedure on the claims submitted for both, the trial and permanent implantation procedures. When the trial is rendered in a setting other than hospital OPD, providers will need to submit a PAR for CPT 63650 as part of the permanent implantation procedure in the hospital OPD.
Prior Authorization Reminder
When submitting a Prior Authorization Request for trial and permanent implantation only one UTN should be requested and used on both claims. If a second Prior Authorization Request is submitted for the permanent placement this will be considered an invalid submission and will be rejected.
