Don’t Forget the CMS-588 Electronic Funds Transfer Authorization Agreement

The CMS-588 Electronic Funds Transfer Authorization Agreement is to be submitted for billing providers upon initial enrollment, change in bank information and during revalidation. 

    • Initial enrollment submit with the CMS-855 application
    • Any type of change in bank information (e.g. change in financial institution, a bank changes routing or account number)
    • Revalidation submission if a current version of the CMS-588 is not on file

Tips

    • Avoid payment holds by verifying National Government Services has sent notification that the new banking information has been set up prior to closing account.
    • Update banking information no more than 60 days prior or 30 days after change.
    • Submit voided check or bank confirmation letter indicating legal business name.
    • Bank account name must match legal business name on IRS document/Social Security record.
