The Reason Code Look Up Tool For Top Claim Errors Tool
The Reason Code Look Up Tool for the Top Claim Errors is available for our Part A providers (Part A, HHH and FQHC) to search for common claim error reason codes including denials, rejections and RTPs.
To locate this tool, select Claims and Appeals from the dropdown.
Select Top Claim Errors.
You can search by keyword, reason code or you can select an Error Type from the dropdown.
Medicare Secondary Payer Related Diagnosis Codes
Recently, we have seen an increase in inquiries from providers attempting to obtain related diagnosis code information for patients with primary insurance to Medicare.
Did you know that you can obtain this information when available via NGSConnex. Located under the Eligibility Lookup tab. If the beneficiary has Medicare secondary payer information available, you can select ‘Medicare Secondary Payer’ from the left-side navigation.
If the primary payer is limiting coverage to specific diagnosis codes, the ICD-10 diagnosis codes will be provided when available.
If the Related Diagnosis Codes field is blank there are no related diagnosis codes available. You must contact the primary payer to obtain any additional information.

How Do I Create an Account on NGS Medicare University?
If you haven’t had a Medicare University account or haven’t used Medicare University within the past two years, you’ll need to create a new account.
To create a new Medicare University account:
- Click “Create Account” on the Medicare University login page.
- Complete the “Medicare University Course Registration” page (all of the following fields must be completed):
- First Name (your preferred first name)
- Last Name (your preferred last name)
- Email (your valid email address)
- Ensure this is typed correctly and is an email account you have access to. This will be your username and you’ll need to access it to complete your registration.
- Confirm Email (same email address as previous field)
- Language (use drop down button to choose your preferred language)
- Code - Type in one of the following to indicate your line of business (LOB): Part A, Part B, HHH, FQHC, ALL.
- If you need access to multiple LOBs, you must complete a separate form/ registration process for each.
- If you need access to all LOBs, you can complete just one form/registration process and type in ALL.
- If you do not type in the code correctly, “Please enter a valid code” will appear in red at the top of the form and all the information you entered will be erased.
- Click the blue “Register” button once all sections are completed.
- You will then be redirected to a page that indicates which LOB the account was registered for (All LOB, Part A, Part B, HHH or FQHC).
- To login to Medicare University, you will need to click the link that is in the body of the email you will receive from Medicare University <system@litmos.com> with “Medicare University login information” in the subject line.
- This email contains your username as well as a temporary password.
- Please note that the verification link can only be clicked once as it will expire after the first use. After this link has been used, login at https://NGSmedicareuniversity.litmos.com.
- When you click the link, you’ll be directed to the Medicare University home page, where you’ll enter your username and temporary password and click the blue “Register” button.
- Complete the three sections of the Confirmation and Additional Information page that loads then click the blue “Complete” button on the top right-hand side of the page.
- Section 1 – Confirm your name. Review that your first and last names are listed correctly (required fields).
- Section 2 – Create a password. Your username is the email address you entered when you registered (cannot be changed). You will need to enter a new password and confirm that password in the appropriate fields.
Note: Passwords must be eight characters in length and must contain:
- One uppercase letter
- One lowercase letter
- One number
- One special character
You can click on the box next to Show Password to ensure that you are typing in the same password in both fields.
- Section 3 – Additional information. These are optional fields which can be completed to assist us in Identifying users. This information will only be used internally by National Government Services.
- Once you click the “Complete” button, you will be redirected to the login page and prompted to enter your Username and newly created password to start using Medicare University.
