CEDI NPI Crosswalk Tool: 

A crosswalk is the link established between the National Provider Identifier (NPI) and durable medical equipment (DME) Provider Transaction Access Number (PTAN) (supplier ID). Verify your link using the tool below. Enter your NPI and DME PTAN (supplier ID) and select Search.  

Note: Your DME PTAN will be a 10 digit number likely ending with 0001, 0002, 0003, etc.

After selecting Search, the tool will indicate if “Yes” the crosswalk link is established or “No” the crosswalk link is not established. If your NPI and DME PTAN is not crosswalked, please take the necessary steps provided to establish this link before completing enrollment forms or sending claims as it will cause rejections.
