Error Description:

The claim contains a vaccine HCPCS code; however, the required condition code A6 was not billed on the claim. Condition code A6 is required when billing for vaccine(s) and/or vaccine administration.

Avoiding/Correcting This Error

Ensure the correct HCPCS code(s) were billed. Add condition code A6 when vaccine-related codes are billed.
