New NGS Part A Redetermination Request Form

Save time and money by using of the following options instead of mailing a written redetermination request form:

• Initiate an adjustment in the FISS DDE for fully covered or rejected claims
• Use NGSConnex to submit your redetermination and include any documentation

If you are unable to use FISS DDE or NGSConnex, then please begin to use the National Government Services Part A Redetermination Request Form – Level 1 form starting today. We will continue to honor requests submitted on the CMS-20027 form.
