1. TOB is 71X and the claim was billed with revenue code(s) other than 052X, 0900 or 0780
2. FQHC or RHC TOB 71X or 77X was billed without the LIDOS
3. TOB 71X or 77X claim was billed with revenue code(s) that are not billable in this setting 002X–024X, 029X, 045X, 054X, 056X, 060X, 065X, 067X–072X, 080X–088X, 093X–031X

Avoiding/Correcting This Error

Ensure the appropriate revenue code(s) and HCPCS/CPT code(s) are reported on the claim and each claim line contains the correct service date. Verify the information billed and PF9 to resubmit.
