Patient status is equal to 30 on TOB 81X or 82X, excluding frequency code A–E or 8, with From date of service on or after 7/1/2013, and the statement To date is not equal to the last day of the billing period month.
Avoiding/Correcting This Error
Hospice claims are required to be billed monthly. Verify the month beginning and ending date and the ‘From’ and ‘To’ dates on your claim. The claim must span the entire month for continuing care claims.
