Provider Enrollment: Change in Practice Location

The fastest way to update a provider enrollment record is through PECOS for the individual (NPI Type 1) or entity/group (NPI Type 2). PECOS allows you to edit, add and delete information according to the update.

Updates can be submitted through either PECOS or the appropriate CMS form.

Changes to the practice location must be report within 30 days and no more than 60 days prior to the effective date of the change.

If a group or individual with a private practice has moved, complete the practice location section to end date the location from the enrollment record and add the new practice location with the effective date you saw the first Medicare patient. Remember to verify all enrollment information, including correspondence address and special payments address.
