Provider Revalidation Interactive Tool 

This tool is designed to assist Part B applicants who are revalidating enrollment. If the reason for your application is other than revalidation, then please refer to the Enrollment tab on our website for guidance. 

There are two methods for revalidating, either using paper applications or the secure Internet-based Provider Enrollment Chain and Ownership System (PECOS).  PECOS is the preferred method since it has advantages that include:

• Automatic selection of proper enrollment form(s)
• Tailored application process
• Fewer submission errors/omissions
