National Government Services: Raising Awareness about Care Management

According to the Center for Disease Control (CDC), an estimated 117 million adults have one or more chronic health conditions, and one in four adults have two or more chronic health conditions.

As your Medicare Contractor for Jurisdiction 6 and Jurisdiction K, NGS is raising awareness of the benefits of care management services for Medicare beneficiaries with chronic conditions.

We have a series of webinars scheduled that offer our provider community information about available care management services, and guidance for proper billing of these services. We encourage you to join us to learn about these important benefits. You may attend as many sessions as you’d like, simply click on the title and complete the registration form.     • Covered voluntary service enabling Medicare patients to make important decisions dictating control over type of care received.
Tuesday, 7/20/2021, 1:00- 2:00 p.m. ETCare Management: Principal Care and Chronic Care Management Services
    • Services covered for patients with one (PCM) or multiple (two or more) (CCM) chronic conditions expected to last at least 12 months or until death of patient.
Wednesday, 7/21/2021, 10:00- 11:00 a.m. ETCare Management: Psychiatric Collaborative Care Management and Behavioral Health Integration
    • Services covered monthly, delivered using Psychiatric CoCM, an approach to BHI, shown to improve patient outcomes in multiple studies.
Wednesday, 7/21/2021, 1:00- 2:00 p.m. ETCare Management: Traditional Care Management
    • Services covered during beneficiary’s transition to a community setting following discharges.
Thursday, 7/22/2021, 10:00-10:30 a.m. ETCare Management: Cognitive Assessment
    • Services typically start with a 50-minute face-to-face visit that includes a detailed history and patient exam. Use information you gather from the exam to create a written care plan.
