Incorrect Medicare Identification Numbers Submitted on Provider Enrollment Forms
Providers/suppliers enrolled in multiple business types (Part A, Part B and/or DME) should be cautious of what identifying numbers are submitted on Medicare enrollment forms.
Identifying Numbers
- NPI – assigned by NPPES
- PTAN – assigned by the MAC (also known as a Medicare identifying number)
Applications Include
Internet-based PECOS application is the preferred method to initially enroll, revalidate and update an existing enrollment. Use appropriate identifying information, whether submitting via PECOS or on the CMS-855 paper form.
- Part A Form: CMS-855A
- Part B Form: CMS-855B, CMS-855I, CMS-20134, CMS-855O, CMS-855R
- DME Form: CMS-855S
- Other forms: CMS-460, CMS-588
Once you determine which form to complete verify the correct identifying number combination(s) are submitted on that CMS form. If no PTAN has been assigned for that business type indicate “pending” in the Medicare Identifying Number field(s) on the application. If you have multiple NPIs, verify the correct number is submitted according to the enrollment scenario.
