There are several ways this can be obtained. A pain scale is a simple and efficient way to collect this information post procedure. A follow up phone call can check on how the patient is doing (any complications) and provide opportunity to get this information. An electronic submission of the information via a portal or encrypted email which would “time stamp” the communication could be done.
