Provider Enrollment Application Fee Amount for CY 2021

On 11/23/2020, CMS issued a notice: Provider Enrollment Application Fee Amount for Calendar Year 2021. Effective 1/1/2021, the application fee is $599 for institutional providers (as that term is defined in 42 CFR Section 424.502) that are:

1. Initially enrolling in the Medicare or Medicaid program or  CHIP;

2. Revalidating their Medicare, Medicaid, or CHIP enrollment; or

3. Adding a new Medicare practice location.

This fee is required with any of these enrollment applications submitted from 1/1/2021 through 12/31/2021.
