Failure to Report Address Changes

Providers who fail to report an address change will result in suspension of Medicare payment.  

All Medicare providers are responsible for providing their Medicare contractor with their current and accurate addresses.

Notification to Medicare can be submitted up to 60 days in advance of your address change but no less than 30 days after address change. This will help you avoid the delay in your payments. Any address changes received in advance of 60 days will be returned.
