Medicare Participating Physician or Supplier Agreement
Download and complete CMS-460 Medicare Participating Physician or Supplier Agreement.
Note: CMS-460 must have a signature and date of participant or authorized representative of participating organization.
You have 90 days from when you initially enrolled to decide if you want to be a participating provider or supplier. Otherwise, you can only change your participation status during the annual open enrollment, which is mid-November through December 31st of each year which will become effective the first of the following year.
Benefits of Medicare Participation
- Medicare reimbursement is five percent higher than for those who do not participate.
- Medicare issues payments directly to you because the claims are always assigned.
- Claim information is forwarded to Medigap (Medicare supplemental coverage) insurers.
Note: Physician and nonphysician practitioner who have employment arrangements or reassigns benefits to a group/individual, claims will process according to the participating status of the group/individual. The Participating Status on Provider Enrollment Approval Letters will reflect the individuals participating status, not the groups.
