Eligible Method II Providers

If a CAH wants to become a Method II biller, email the request to A&R at ngsreimbursement@anthem.com. 

Suppliers who can enroll as a Method II Biller Reassignment.

  • Doctor of Medicine (MD)
  • Doctor of Osteopathy (DO)
  • Doctor of Chiropractic (DC)
  • Doctor of Dentistry (DMD) (DDS)
  • Doctor of Optometry (OD)
  • Doctor of Podiatry (DPM)
  • Marriage and Family Therapist (MFT)
  • Mental Health Counselor (MHC)
  • Nurse Practitioner (NP)

Individual provider must have an active enrollment (CMS-855I) to reassign to a Part A Entity (e.g., critical access hospital – CAH II)
