Deactivation, Suspension and Reactivation

Failure to Report a Change in Bank Account Information

Providers who fail to report a change in bank account information will result in suspension of Medicare payment and termination of Medicare billing privileges.

All changes initiated by either the bank or the provider must be reported to Medicare within 30 days of the effective date of the change by submitting a CMS-588.
