Deactivation, Suspension and Reactivation

Revocation

CMS may revoke a currently enrolled provider or supplier's Medicare billing privileges and any corresponding provider or supplier agreement for the following reasons:

  • Abuse of billing privileges
  • Failure to document or provide CMS access to documentation
  • Failure to report requirements
  • False or misleading information
  • Felonies
  • Grounds related to provider and supplier screening requirements
  • Improper prescribing practices
  • Initial reserve operating funds
  • Medicaid termination
  • Misuse of billing number
  • Noncompliance
  • Onsite review
  • Prescribing authority
  • Provider or supplier conduct

A revocation letter will be sent to the correspondence address on your enrollment record, please follow instructions within the letter.
