Revocation
CMS may revoke a currently enrolled provider or supplier's Medicare billing privileges and any corresponding provider or supplier agreement for the following reasons:
- Abuse of billing privileges
- Failure to document or provide CMS access to documentation
- Failure to report requirements
- False or misleading information
- Felonies
- Grounds related to provider and supplier screening requirements
- Improper prescribing practices
- Initial reserve operating funds
- Medicaid termination
- Misuse of billing number
- Noncompliance
- Onsite review
- Prescribing authority
- Provider or supplier conduct
A revocation letter will be sent to the correspondence address on your enrollment record, please follow instructions within the letter.
