Revalidation Request Mailed
We will mail the revalidation notices to identified providers and suppliers currently enrolled in the Medicare Program. The revalidation notice will be sent two to three months prior to due date.
Each provider or supplier will receive their own revalidation notice, mailed in a yellow envelope.
- Mailing will be sent to the special payments address and to the correspondence address.
- If both addresses are the same, a second notice will be mailed to a provider’s practice location address.
- If all three addresses are the same, only one notice will be mailed.
Important Notes
- Due date is stated on mailed notice
- Failure to submit by due date or not responding timely for additional information will result in your Medicare claims being rejected or deactivation of Medicare enrollment.
- Revalidation application received more than seven months prior to the due date are considered unsolicited applications and will be returned.
