Select Medicare Diabetes Prevention Program from the left-side navigation.
The Medicare Diabetes Prevention Program expanded model is a structured intervention with the goal of preventing type 2 diabetes in individuals with an indication of prediabetes. Medicare beneficiaries who are currently enrolled in Medicare Part B and meet specific clinical requirements are eligible to participate in the program.
Deductible and Co-insurance is not applicable for Medicare Diabetes Prevent Program services.
