Approval or Denial

Once your affidavit is approved, you will receive an approval letter of opt-out status along with the opt-out effective date.

Review this letter to ensure that everything is accurate and if you have any questions, please contact the Provider Enrollment Contact Center.

Affidavits will be denied for nonresponse to development requests.
