The GPO is responsible for the production and distribution of information products and services including the official publications of Congress, the White House and other federal agencies in digital and print formats. GPO provides for permanent public access to Federal Government information at no charge through our Federal Digital System website.
The CMS-1500 claim form is the standard health insurance claim form used by a noninstitutional provider or supplier to bill MACs and DME MACs when a provider qualifies for a waiver from the ASCA requirement for electronic submission of claims.
The NUCC is responsible for the maintenance of the CMS-1500 claim form; however, claims can be purchased from the GPO via mail, telephone or internet.
